Patch 12.1 has an unusually high concentration of special effect gear — cantrip weapons for nearly every class, a new armor mini-set, jewelry with a raid-specific twist, and a long list of trinkets across the raid and the eight-dungeon rotation.
Two things about this batch are worth understanding before you start chasing any of it:
The best cantrip weapons drop above the normal upgrade track on Mythic, because most are Very Rare or come from the final two bosses. That makes them near-guaranteed best-in-slot for the season.
Most cantrip jewelry only works inside one specific context, which makes several of these pieces far less valuable than their tooltips suggest.

Seven of the ten cantrip weapons come from the last two bosses of the raid, and four of those are Very Rare drops. That concentration matters for planning: if a cantrip weapon is your target, your entire path runs through The Coiled Altar and Ula'tek.
One thing worth knowing about the agility dagger: it's locked to main hand. There's no double-proc setup available, and Heroic versions will be straightforwardly outclassed by Mythic main hands rather than finding a home in the off-hand slot.
Cantrip Armor — Ula'tek Only
All four special effect armor pieces drop exclusively from Ula'tek, the final boss.

How they work: each piece carries one secondary stat instead of two. A proc further increases that stat while reducing all others by a smaller amount.
That design creates a genuine decision. A single-stat piece with a proc is excellent when the stat is one your spec heavily favours, and mediocre when your spec wants a balanced spread. Check what your spec actually values before assuming a cantrip piece beats a standard drop at the same item level.
The Catalyst Interaction Worth Knowing
Catalyzing cantrip armor retains both its secondary stats and its special effect.
This is the most useful mechanical detail in the whole batch and it's easy to miss. You can convert a cantrip armor piece through the Matrix Catalyst into your tier set piece, and it keeps the proc.
What that means practically: a cantrip piece isn't a choice between the special effect and your set bonus. You get both. If you loot one of these four and you're missing that tier slot, converting it is strictly better than converting a plain drop of equal item level.
Cantrip Jewelry — Read the Caveat First
This is where the batch gets misleading.
Most cantrip jewelry provides damage reduction against Deadly Venom — a mechanic found in The Venomous Abyss and its surrounding outdoor zone, and nowhere else.
Outside that content, these pieces offer no special advantage.

There's also no indication that wearing multiple stacks the benefit. Two rings with the same effect likely won't double your protection.
How to treat these: they're raid progression tools, not general best-in-slot items. Valuable while you're learning The Venomous Abyss and taking heavy Deadly Venom damage. Considerably less relevant once the raid is on farm, and irrelevant in Mythic+ entirely.
Ula'tek is the exception. The Aqirbane Reliquary sits outside the Deadly Venom pattern, which adds to an already long list of reasons the final boss carries the most valuable loot table in the raid.

Altar of Fangs and The Blinding Vale each carry four trinkets — the densest trinket sources in the dungeon pool. If trinket upgrades are your priority, those two dungeons deserve more of your key rotation than the others.
